Application Materials

Application for Undergraduate Admission (PDF, 365K)
For freshman, transfer, and reentry applicants. Includes the application, personal statement section, and high school counselor signature section.

International Application for Undergraduate Admission (PDF, 77K)
For students who are neither citizens nor permanent residents of the United States. International applicants must also submit the Financial Verification Form (PDF, 191K). Review the International Application Checklist (PDF, 78K) for complete application details.

High School Counselor Form (PDF, 39K)
For high school counselors to complete as part of the application for admission. The counselor signature section is included on the paper application and is presented as a printable form when completing the online application. This downloadable form is an exact duplicate of these print and online counselor signature sections and can be used when the other versions are unavailable.

Recommendation Form (PDF, 32K)
The use of this form is optional. Recommenders may complete this form or simply submit a letter of recommendation on letterhead, plain paper, stationery, etc.

Financial Verification Form (PDF, 191K)
For international applicants. This form is a required component of the application for admission and documents the student's ability to pay all tuition and living expenses for their time of study at Wisconsin.

Freshman Request for Deferral (PDF, 52K)
Strongly qualified admitted domestic freshman students may request deferred admission by completing this form.
